What To Expect At Your Appointment
Your First Visit
When you arrive to the clinic, you will be greeted by the friendly staff at The Physical Therapy Place. Our clinic is a comfortable, warm, and safe space.
- If you are a new patient, please arrive 10 minutes early to fill out the proper forms and paperwork. You may click on the forms link below to print forms and complete in advance. Please bring them in with you when you arrive.
- Please bring your insurance information with you to the first appointment. If you are using Workman’s Compensation, please bring in the claim number and case manager information.
- If required by your insurance, please have your doctor fax your referral to us at (907)569-5562. If you have a copy of the referral, please bring it with you.
- Bring in any test results or specialist reports you have.
- Dress comfortably. If during your appointment you need comfortable gym clothes, you can borrow the clean clothing that we have available in the clinic.
Appointments last for 45 minutes. During this time you will receive one on one time with your Physical Therapist. Your appointment will take place both in a private room and the clinic gym, depending on the specific treatment for your condition. Our therapists will work with you to develop a comprehensive treatment plan for your recovery.
Following Visits
Using the mind and body in unity is an active part of your recovery
Subsequent visits are 45 minutes in length. The frequency of your appointments and duration of your treatment will depend on your condition. Joy and Katie will work with you to understand your problems and will involve you in your active treatment plan. The more committed you are to your recovery, the faster you will progress.
Insurance
Most insurance plans are accepted, including the following:
- Alaska Care
- Medicare
- Medicaid
- Premera
- Tricare
We are preferred providers for the following:
- Aetna
- Cigna
- Blue Cross Blue Shield
We accept and bill:
- Workman's Compensation
- Denali Kid Care
- Medicare, Medicaid
We also accept self pay. Please call if you have any questions about your insurance.
Referrals
Referrals are accepted from physicians and nurse practitioners. In Alaska you may see a physical therapist without a referral. Some insurance companies, however, require a referral.
Cancellation and No Show Policy
We require 24 hours notice for cancellation of your appointment so your time may be filled by someone on the waiting list. If you cancel your appointment with less than 24 hours notice, a cancellation fee of $25 will be charged . If you do not attend an appointment and you do not call to cancel or reschedule, you will be charged for the appointment.
