The Physical Therapy Place is a preferred provider for many insurances including:

• Aetna

• Blue Cross Blue Shield

• EBMS

Other Plans

Most insurance plans are accepted, including:

• Aetna- including the union plans such as ASEA, Teamsters, Local 52

• EBMS

• GEHA/Aetna Signature Administrators

• Medicare

• Medicaid (Denali Kid Care)

• Meritain/Aetna Choice Point of Service 2 

• Premera Blue Cross Blue Shield

• Project Access

• Tricare

• Workman's Compensation

We recommend you call to check if we are a preferred provider for your specific insurance plan. 

Referrals

Alaska is a direct access state, this means you may see a physical therapist without a referral. Some insurance companies, however, require a referral.

Referrals are accepted from physicians and nurse practitioners.

Fees for Service

Evaluation (97161 / 97162 / 97163)               $210 
Therapeutic Activities (97530)                       $70/unit
Therapeutic Exercise (97110)                         $70/unit
Manual Therapy (97140)                             $70/unit

If you do not have insurance or wish to pay cash, we also accept self pay. Please call our office to discuss this option.

No-Show & Late Cancellation Policy

Please give 24 hours notice for cancellation of your appointment so your slot may be filled by someone on the wait list. If you cancel your appointment with less than 24 hours notice, a $40 cancellation fee will be charged. If you do not attend an appointment and you do not call to cancel or reschedule, you will be charged $80 for the appointment.
